Certificate Holder & Additional Insured

Submission Instructions

Submit a current, signed COI to your Wahl Sourcing representative at onboarding and upon each policy renewal. Expired certificates are not acceptable — submit updated certificates before policy expiration.

Required ACORD 25 Fields

  • Producer: Your insurance agent/broker name, phone, and email
  • Insured: Your company name and address
  • Insurers: Name and NAIC numbers for each insurer
  • Policy Numbers & Dates: Effective and expiration dates for each coverage
  • Limits: Must meet or exceed WCC minimums above
  • Description of Operations: Must include the Additional Insured language as specified above
  • Authorized Representative Signature
Note: A Certificate of Insurance confers no rights upon the certificate holder. It does not amend, extend, or alter the coverage afforded by the policies. Review actual policy documents for coverage terms and exclusions.
