Purpose & Scope

This form documents that the supplier's authorized representative has received, reviewed, and understood the Wahl Clipper Corporation Supplier Quality Agreement — Sterling (QA-001 v1.0) and commits the supplier organization to compliance with all requirements contained therein. This acknowledgment is required for all suppliers shipping product to the Wahl Clipper Corporation facility in Sterling, Illinois.

By signing this form, the undersigned authorized representative confirms on behalf of the supplier organization that:

(1) The supplier has received and reviewed the Wahl Clipper Corporation Supplier Quality Agreement — Sterling, Document QA-001, Version 1.0;

(2) The supplier understands the quality requirements, obligations, and processes defined therein; and

(3) The supplier commits to full compliance with all requirements in QA-001 for any product supplied to the Wahl Clipper Corporation Sterling, Illinois facility, effective from the date of signature below.
Confirmed Understanding — Key Obligations

Check each box to confirm understanding of the following requirements. These items do not replace the full QA-001 document — they highlight obligations commonly requiring supplier action.

Documented Quality System. Supplier operates a documented quality management system capable of consistently meeting Wahl's specifications. ISO 9001 is the recognized reference standard. QA-001 §2 — Quality Management System
PPAP / Part Submission. Production Part Approval Process (PPAP) approval is required for all new and changed products. Supplier will not ship production parts without a Wahl-approved Part Submission Warrant (PSW). QA-001 §8 — Sampling / PPAP & Appendix
Critical to Function (CTF) Characteristics. Supplier understands that CTF characteristics are called out on Wahl engineering prints and require elevated process capability (Cpk ≥ 1.67) and SPC in serial production. PSW will not be approved without CTF characteristics validated. QA-001 §4 — Critical to Function Characteristics
Engineering Change Approval. No change to product, process, material, sub-supplier, or manufacturing location will be implemented without prior written approval from Wahl. QA-001 §5 — Changes Requiring Wahl Approval
Nonconformance Response. Supplier will respond to Wahl-issued NCRs within the timelines defined in QA-001, including containment, root cause analysis, and corrective action. Charge-backs may apply per SRC-001 Addendum A. QA-001 §7 — Supplier Defects & §13 — CAPA
Quality Performance Standard. Supplier acknowledges the incoming rejection rate target of <300 PPM and the in-process capability requirement of Cpk ≥ 1.33 (Cpk ≥ 1.67 for CTF characteristics). QA-001 §9 — Incoming Inspection & Wahl Scorecard
Wahl Audit Rights. Supplier agrees to permit Wahl or its designee to conduct quality audits of supplier facilities and processes upon reasonable notice. QA-001 §14 — Supplier Audits
